Give Codex the plugin's icon, brand colour and descriptions - #46

Codex now installs this repository as the Primitive plugin, but the manifest carried no Codex presentation data, so the plugin showed up with a lowercase name, no icon and no brand colour. Add the icon and an OpenAI extension block to plugin.json, and have CI fail if the block or the icon it points at goes missing. Why
Since primitivedotdev/codex-plugin#1, Codex installs this repository as the Primitive plugin. The old Codex-only manifest (display name, descriptions, brand colour, default prompt) and icon were dropped in that change, and nothing here replaced them. Codex currently falls back to the plain manifest fields: lowercase
