Adds one bullet to the CLAUDE.md decomposition standing rules:

A merge audit on 2026-08-27 found 31 merged PRs had silently left their issues open (no closing keywords in PR bodies), and two coders escalated base mismatches as blockers that were normal sequential-merge behavior. Fleet-wide directive already delivered to all live sessions; this makes it durable for every future coder (CLAUDE.md is read from each task worktree at session start — no daemon restart needed).
Docs-only: one bullet in CLAUDE.md. No code, no migrations. (Body corrected: an earlier draft carried a stray Closes reference to an unrelated issue — this PR has no mapped issue, like #3064.)
