ForTokenAfterRemoval checked that the owner's reader was still registered, then ForToken looked it up again to read its override. When the exit timer fired while the reader was disconnecting, the first lookup could see it and the second miss it, so a hold reader on a tap device fell back to the global tap mode and the media it launched never exited. Both now resolve from a single lookup. TestTimedExit_KeepsHoldPolicyWhenOwnerReaderDisconnects hit this on CI: with the default exit delay of 0 the timer fired as it was armed, racing the reader removal the test meant to happen during the delay. It now sets a delay so the removal lands while the timer is pending, and a scanmode test drives the disconnect between lookups directly.
